I spent this morning/afternoon calling several CS offices and driving to 5 different physical CS offices to talk in person and none of them would do it, or they wanted to do a medical first and wouldn't take the instructions saying I don't need another one. My plan is to send in my DS3025 that is completed (the box that says "US vaccination requirements COMPLETE (requesting a blank Waiver)" was checked off), and send in my immunization history from my previous family doctor back home.

Also, for a job back home I had to have TB skin test and a Hep B test done, which happened in April, a few months before I moved to the US (July 29)... should I include those documents a supporting evidence as well?

I do not have an original Form I-639 unless the doctor I saw for my visa medical did one?

I will also include a cover letter with an explanation of why we don't have the I-639?

Do I have to file a Form I-130 as well and pay the fee? Along with my AOS package?

Those adjusting from a K-1 do not need an I-130. You must include a copy of your NOA2 I-129F approval notice instead. Follow the I-495 instructions and guide here on VJ. They list what is required.

Hey guys, thanks everyone for the help, I finally have my AOS package complete and ready to mail, here is what it looks like from my cover letter.

Does anyone have any comments of advice?

For the Civil Surgeon, I read on here I didn't actually need one but for piece of mind I went ahead and had it transcribed and signed. He wouldn't fill out Part 3, and wouldn't include anything but Part 7 (Vaccination information) so I had to take what he gave me.

We only used paper clips. Also, make sure you make at least one full copy of the entire package for your records. We made three, but I am a little paranoid. When we had our interview I took one copy of the AOS along with a copy of the original K1 package with us which was good as the IO asked for some records again.
