Hi all,

Not sure if I am in the right forum topic but I am trying to post this through my phone. I tried looking for a few of these questions, but didn't have much luck or was sent in a few circles, especially when it came to the government sites. I have a few questions and need some clarifications as I have entered the U.S. on my K-1 and got married, and am now looking to file my AOS.

1) When I submit the Form I-485, am I missing any accompanying documents that should be with it? So far I have a copy of my birth certificate, copy of K-1 visa, 2 passport photos, copy of marriage certificate, and a copy of my I-94 Arrival/Departure document.

*** 2) Do I need my full medical examination record or just vaccination report to accompany my I-485 as I am not sure that I have all of that?

3) Can I make the payments all on one check or do they have to be separate? And do I include payment for the biometrics even though I have to wait for them to get back to me on that?

4) Here are the forms I will be submitted as well as the associated feesI am looking at fees, am I wrong on any of them?:

I-485 $985 (form), $85 (biometrics),

I am confuse with the Medical Examination Report that I have to send in with my I-485, I do not have this, it was in the sealed envelope and the Border Agents kept it to forward it somewhere. Do I need to get another copy or? I have my chest xray on a disc, do I do anything with that? THANKS!

Should just have the 3025 vaccination record. If it is complete you should be all set. If not, or if your case drags out for more than a year, you will need to find a civil surgeon to complete the vaccines, or do a complete physical.

One check is fine.

Did you get a copy of the G3025 vaccination record when you did your medical? You should have along with your x-rays.

I didn't get a G3025, everything was in the packet I got that I could not open and gave to the border agents. I did not get anything back, the agent said he had to send it somewhere or do something else. I have vaccination records from home but it's not on any US forms for anything.

Since the medical report was in the packet, do they send it off anywhere? Do I have to still send in anything medical?

Not sure about Canada, but Katya received her 3025 and a copy of the x-Ray from her physical. Did you check the disk to see if the 3025 was there? Take a look at number 11 in the guide. It is a little confusing, but if the vaccines are not complete it will almost always result in an RFE.

I haven't gone through my POE yet, by my K-1 was approved last week in Montreal. I did my medical at Medisys in MTL, and I received the xray in a brown envelope, the medical I submitted at the interview, and then the vaccination records as a separate sheet.

Hubby did POE in February, they took the medical disks and whatnot out of its package, but no idea if they sent it off or if it collects dust in their desks. Regardless, we haven't been in possession of the information since POE.

When Hubs did his medical in TO, he was given a DS3025 by the doctor which was outside of the packet and received the day he did the medical (i.e. before he got the packet). If you're missing the DS3025, you'll need to find a Civil Surgeon in your area after you POE who will a) transcribe your vax record b) not rip you off. We sent in the DS3025 with an explanation in the cover letter why we didn't send in a 693 and they seemed to be happy with that since his GC is in the mail.

Here's the thing, you can submit without the I-693 and explain you had a valid medical performed on XYZ date and send a copy of your vax record if you want to try and save the money. They are supposed to have a record of your medical from your K1 process, but this is USCIS, so they might not have it. If they find your Vax copy insufficient or if they don't have that medical on file, you'll have to do the Civil Surgeon route anyway and you'll either send that in a RFE response or bring it to your interview (varies by person/officer).

In the meantime, I'd rummage through your K1 stuff and see if you can find it. I didn't even realize that the receipt Hubby got from the doctor was an official DS3025 until after we sent the copy in.

If your 3025 is complete, you should be all set since you are filing from a K1. Don't worry about a CS as this point since your original 693 should already include the vaccine record. If there is a problem with USCIS matching your K1 693, then you may need to look for a CS. I agree with the CS issues, with Katya, her 3025 was not complete, so we had to find a CS and they required a complete physical as well as the added vaccines. Luckily, it only cost us about $250 total, but should have been more like $50 for just the shots.

I spent this morning/afternoon calling several CS offices and driving to 5 different physical CS offices to talk in person and none of them would do it, or they wanted to do a medical first and wouldn't take the instructions saying I don't need another one. My plan is to send in my DS3025 that is completed (the box that says "US vaccination requirements COMPLETE (requesting a blank Waiver)" was checked off), and send in my immunization history from my previous family doctor back home.

Also, for a job back home I had to have TB skin test and a Hep B test done, which happened in April, a few months before I moved to the US (July 29)... should I include those documents a supporting evidence as well?

I do not have an original Form I-639 unless the doctor I saw for my visa medical did one?


I will also include a cover letter with an explanation of why we don't have the I-639?

If your 3025 is complete, then don't worry about sending a copy, you can send one if you want. USCIS should already have a copy of the 3025 and 693 from your K1. Just hold onto it since it is complete, and if for some reason the local USCIS office does not get your medical package then you have the complete 3025.
