Hey ECUK1,

I didn't have a flight number nor flight date on the notarized document as at the time I wasn't sure when we would be travelling however I would suggest as your child is a follow to join to add this information onto yours and a contact number for yourself and/or the child's father, depending on the airline they might want to verify the document especially if the child's father is not taking the child to the airport.

Another suggestion would be to ring the airline as they seem to have different policy's on what the travel document should contain, I found British Airways to have very strict policy's when it came to going abroad with minor children while Qantas didn't.
