My fiance and I are getting married this week. The 90 days are almost up. Now I'm extremely confused because I'm not sure if you have to send out the permanent resident papers out as well before the 90 days or it's just that you have to be married before the 90 days? Please help!!!!! Time is ticking

Nope, just make sure you marry within the 90 days. You can send out the adjustment of status papers later on. Your fiance with technically be ~out of status~ but, as soon as you file the AOS petition, they'll be legally allowed in the US until a decision has been made on the petition.

Oh god thank you!!!! Makes me feel way better. Also the permanent resident papers they are asking for vaccination are those required or is it a choice even though he already had his analysis done in the Dominican republic

Nope, he doesn't need a new medical if he completed it within 1 year of his overseas medical exam.

Did he get all of his vaccinations at the medical in DR? If not, then he'll need complete the vaccine portion of the Form I-693 by seeing a civi surgeon (doctor authorized by USCIS).

From the Form I-693 instructions (page 6-7):

"Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the Part 7. Vaccination Record completed by a designated civil surgeon. In this case, you must submit Parts 1., 2., 4., and 7. of Form I-693."

im k1 visa i am with my k2 in the Us now do i need to get separate form for I 485?

They both need to file for AOS, and both need their own forms, I-485, I-765 and I-131 > Special Instructions show fees > https://www.uscis.gov/i-485
