Sorry to repeat my previous questions regarding my AOS paperwork but I am having a hard time with it.

So, here I go:

1) EAD: Question 9 - It asked to put all my Social Security Numbers.... Do I write my Canadian number (it is the only one I ever used)?

2) Travel Doc: Part 3 - It asked for a specific travel date.... which I do not have. Do I leave it blank?

Part 7 - Multi-visit box option.... this is what I want, but I'm confussed with part 3.

3) Allien Registration #: I can't find it. The only "A" number that I was able to located was on my I-129f NAO2 letter. BUT, it is a 9 digit number, and the VJ example form has only 8. Is this normal? Where can I locate the number?

4) Medical Examination: I tried to read the threads that deal with what we actually need to do BUT it is so contractory. Can anyone please shed some light on what exactly I need to do? Also, where can I find a Civil Surgeon in Chicago?

5) Marriage Certificate: Is a copy o.k., or do I need to send the original?

MANY thanks for all you help. I greatly appreciate any guidance, as I have no idea what I am doing!

  1. NO, leave that blank (your SIN is NOT recognised as SSN)
  2. Just put in some shaganappi date, and check "multivisit option"
  3. yes, the A# off your NOA-2
  4. Civil Surgeon locator (type in your zipcode and run)
  5. copy is OK (DO NOT send originals to USCIS)
