I am sending off my husband's paperwork tomorrow for concurrent filing of I-130,I-485 and I-765. But I am so confused as to where the cover sheet goes and how to seperate the forms. I know I can paperclip the forms, do I put everything in a box with the cover sheet outside? I have looked everywhere to find picture of example of how to file, but I have had no luck.

Please, anyone with any insight would be a blessing.

The way I filed mine for my wife and two stepdaughters was this way (in order):

G-1145 - Form for E-notification of Application/Petition Acceptance (should always be on top)

Cover Sheet

Sheet of paper with check for $1,070.00 payable to U.S. Department of Homeland Security taped to it
I-485 - Application to Register Permanent Residence or Adjust Status
G-325A - Biographic Information, including residence and employment attachment
Sheet of paper with (2) Passport-size photos inside a small plastic bag taped to it
I-864 - Affidavit of Support Under Section 213A of the Act, with accompanying verification paperclipped with it (e.g. ITR, tax transcripts, checkstubs, etc)
I-765 – Employment Authorization
I-131 – Application for Travel Document
Copy of DS-3025 - Vaccination Documentation Worksheet
• i-693 - Report of Medical Examination (SEALED!), if applicable
• Copy of I-797 - Notice of Action, Approval of K1 petition
• Photocopy of Applicant’s Passport biographical details page
Photocopy of Applicant’s K-1 visa in Passport
Photocopy of USA entry stamp on Passport
• Copy of Applicant’s Electronic I-94
• Copy of Applicant’s Birth Certificate
• Copy of Marriage Certificate
• Copy of Divorce Decree, if applicable

06/10/2015 - AOS Interview (Result: Approved)

06/18/2015 - Conditional Green Card Received

ROC Timeline:

05/15/2017 - I-751 Submitted 
05/26/2017 - I-751 NOA Received 
06/13/2017 - Biometrics Appointment Completed

07/18/2017 - Extension NOA Received (twelve month extension)
03/05/2018 - I-751 Received by Local Office
04/20/2018 - Infopass Appointment (Result: I-551 Stamp on Passport, six-month extension)

06/18/2018 - Extension NOA Received (eighteen month extension)

10/29/2019 - Infopass Appointment (Result: I-551 Stamp on new Passport)

11/06/2019 - I-751 Approved

11/14/2019 - Permanent Green Card Received

I did ours with a cover sheet for each petition, I-130 and another for the I-485

showing in order what was with each petition.

Did a main coversheet on top of it all to show what all was in the box we used to send the AOS, as we are adjusting from a Tourist Visa and 3 years of marriage. Including the $1070 check for AOS.

Best wishes on your journey, I see your in Houston too, they are not know for fast processing.

I-797C Receipt received:  11-27-2023

Sent: 9/12/16: I-130 + I-485 + I-765 (USPS)

Delivered: Sept. 15th 2016 to Chicago Lock Box

Interview Feb  21st, 2018 for I-485

Interview  May 13th, 2019 for I-130 Stokes interview ( 2 minutes)

NOID issued May 17th 2019

June 5th,2019   USCIS received my response on the  NOID// Addressed the NOID myself, No lawyer ever used in case.

July 1st, 2019  10 YEAR GREEN CARD APPROVED

July 5th, 2019   Approval letters for I-130 & I-485 received in the USPS  mail.

July 11th 2019   Green Card in Hand
