BC and MC - yes, certified original copies in English is necessary.

DS-3025 would be a copy (if you have it).

If it isn't difficult, it isn't worth it.

No no no. Do not submit any originals to the USCIS. Only submit photocopies. Do submit the vaccination worksheet but keep a copy of it.

Don't be like me!

I sent my original Birth Certificate AND original divorce decree when I was sending our K1 package. Then I didn't have it to make copies of, so I had to order a new ones. Big money waster LOL.

But yes, send photocopies. In our cover letter at the bottom I put that all parts of the AOS package were photocopies and that we could provide the originals at their request.
