Hello....I've been reading about submitting Form I-693 when you apply for AOS. This medical information should already be included with your approved

I-129F K1 package, no?

I'm a little confused about why this form is necessary, especially when USCIS already has this information in the K1 package. All of my fiance's (now wife)

and her daughter K2 had their medical exams done in Lima, Peru back in February of this year. The forms are written in Spanish. Do I really need to file

this additional form to complete the AOS package? I'm in the process of assembling my forms and supporting documents now, plus 2 additional copies, just

in case.....

Ah. After reading further into this, I have discovered that if you are a K nonimmigrant Visa holder, and are filing within one year of your medical exam....one does NOT have to do another

medical. However, if the DS 3025 form was not filled out correctly, then you have to prove that you received the required immunizations, and that is where the civil surgeon comes in.

Then you must submit Parts 1,2,4 and 7 from Form I-693. What a hassle, ugh!

I sure hope DS 3025 was filled out correctly......crossing fingers. I'm guessing the DS 3025 was filled out by her doctors during the medical exam portion of the K1 visa approval process?

Am I right on this? Thoughts and comments appreciated as always!

has been filled out correctly or not. My wife says she received many injections and her daughter received at least 7. These vaccinations were given for

entry into the US. I'm thinking they both received the required vaccinations, otherwise they wouldn't have been admitted under the K1....thoughts?

The doctor should've given her a copy of the ds3025. If she had it, she would know if it was complete or not.

It's different from country to country and sometimes from Doctor to Doctor. Some doctors will choose to give all vaccines needed so the person won't need them here. For the k1 the vaccines are not really required, but they are for the aos.

You could always wait to see if you get an RFE in the first place. If you do not have a copy of their DS3025 (usually the doctor should give you at least a copy), you can't include it with your AOS packet to nudge the officer to go looking for the medical documents. If you didn't want to risk an RFE, you could go to a civil surgeon and get the vaccination supplement recertified/completed and include it in your AOS packet (make sure you include the instructions which highlight about what a K1 should be sending in just in case the officer reviewing isn't familiar).

The problem I see here, is since you don't have a copy of your DS3025, you have no proof to show the US civil surgeon that you had immunizations to begin with, so if your wife and daughter received an RFE or wanted to be proactive, correcting this issue is going to be difficult. Is there a way you can contact the overseas doctor you used and request they email your copy of DS3025?

I have located BOTH of the DS 3025 Forms, and they are filled out correctly. They were attached to the back of her medical results from Lima. The box checked says "US vaccination requirements COMPLETE"

So this means everything should be fine....I will make copies of these and send them in with my AOS package! :thumbs:

That's good news. I would photocopy your copies and send it with the AOS. Keep in mind even if you should do this, an RFE can still happen. Some officers simply don't know the rules, or things go missing for whatever reason. If you should receive an RFE, responding to it will be easy, because you can go to a US civil surgeon to get it transcribed/recertified. Even though we sent in our DS3025, we ran into that scenario, where the officer either didn't know what they were doing or they lost the copy.. there was no explanation. We responded to the RFE with the required parts of the I-693.

Finding a US civil surgeon who will just do a transcription can be fussy though, but there is a thread on the forum with a list of doctors who will do it for cheap.

Good to know! Fortunately, there are civil surgeons who are close by where we live....in the event of an RFE. :P
