Vacination record question - Came in on k1

Hi everyone,

I have a query about my vacination records for when I apply for my Adjustment of Status.

When I had my medical done in the UK in April, I was all up to date apart from a tetanus shot (as my last one was more than ten years prior). They didn't give me the shot at the time as I was feeling unwell and was on antibiotics. I was then given my vacination sheet where she had ticked off everything I have had had done but told to hold on to this as I would need this for the AOS.

A month later I had the tetanus shot at my and I have a print off as proof of this, so I'm now all up to date.

However, I'm not sure whether I would need to see a civil surgeon or not. Or would I send that vacination record in with my doctors print off showing I've had it done?

Could someone help me out on this?

Yes a civil surgeon has to sign you off because you were not signed off as complete by the panel physician (Knightsbridge). USCIS does not accept things from your doctor, but a civil surgeon will. Go to a civil surgeon with your DS-3025 vaccination sheet and your doctor's print out, and he can copy them all to the official form (I-693) and sign off.

You do NOT need a full medical exam. Ask all the details before picking a civil surgeon. Many will insist you need a full medical exam. Find one who will do the immunization part only.

What parts of the I 693 do they need to fill out? Would it just be 4 and 7?

It's in the I-693 instructions...top of page 7.

That's helpful thank you, I'll bring a copy with me when I see the civil surgeon as well.

Yes. Notice in the instructions you fill out Part 1 but wait to sign in front of the civil surgeon.
