We applied for my wife's I-485. Because of all the previous immigration papers (K-1 visa, and all the supporting documents) we used her maiden name on the I-485 application. We were afraid to use her new married name because we did not want to confuse the USCIS. She is now approved and has an I-797 (notice of action) with approval for permanent residence. However, it has her maiden name. How difficult is it to have the USCIS re-issue a new residence card with her married name?

Not a problem.

File form I-90 and pay a fee ($260), the correction is not due to USCIS error.

To avoid that fee you needed to file I-485 with married name and madien name noted on the line in section 3A "Give name that is on form I-94"
