DCF Manila - Passport Photocopy

Hi everyone! I have a question, I will be filing DCF in Manila for my husband and kids (they don't qualify for the CRBA) this week and I'm already preparing the documents. I've read several entries here and I just want to clarify some things.

1. Do I file the G325 or the G325A?

2. Is that form only for my husband and me or do I need to fill out for my kids too?

3. Do I need to photocopy all pages of my passports and my kids passport?

Hi there,

1. The G325 is for you (the petitioner) and the G325A is for your beneficiary (or beneficiaries)

2. I believe you'll need one G325A for each beneficiary.

3. Photocopy of passport's bio page (for you and beneficiaries)

Also, don't forget to include all documents that show your relationship to the beneficiaries (ie. Marriage Certificate, Birth Certificate, etc)
