AOS Cover Letter with list of enclosures. (No DS-3025)

Hello: Here is my wife's (former K1 fiance) AOS submission. Kindly review this cover letter and let us know if we missed anything.
Please note: She had about 11 vaccinations at her pre-K1 interview medical exam. Her household belongings (including we believe the DS-3025) are on a boat right now crossing the pacific. Is it true that we don't need any medical stuff if USCIS already has it from the packet they took from her at point of entry?

Enclosed please find my Form I-485, a Form I-864 Affidavit of Support completed by my husband, Eric Applebaum, as well as other forms, documents, photographs, tax return documents, Certified Marriage Certificate and other documents, Form I-765 and supporting documents related thereto: All listed below in this cover letter.

The Contents Include:

- Form G-1145 (e-Notification of Application/Petition Acceptance

- Payment in the amount of $1,070.00 by personal check made payable to “U.S. Department of Homeland Security”. (Payment represents $985.00 filing fee for Form I-485, plus Biometrics Services fee of $85.00, for a total of $1,070.00)

- Form I-485: Completed, signed and dated. Attached thereto in a plastic bag are two Passport sized photographs of myself with my name written on the back of the photographs and with my name on the attached plastic bag.

- Certified Copy of my Birth Records from South Korea with attached English Translations. (NOTE: There are two documents which together function as my Birth Certificate, a “Family Relation Certificate” and an “Identification Certificate”. IMPORTANT: Original OFFICIAL stamps and seals of the Republic of Korea are located on the back of the Family Relation Certificate and Identification Certificate. Additionally, the English translations of each of these two documents are certified and INCLUDE TRANSLATION OF THE ENTIRE DOCUMENTS, INCLUDING THE OFFICIAL STAMPS AND SEALS located on on the back of the original Republic of Korea documents.

- Copy of my Passport Biographical Page.

- Copy of my K1 Visa in my Passport

- Copy of entry stamps from my Passport.

- Form G-325A, Biographic Information. Signed and dated.

- I-864 Affidavit of Support, signed by my husband, xxxx Applebaum. Related to the I-864 Affidavit of Support are the following supporting evidence:

Birth Certificate of my husband, xxxx Applebaum, establishing that he is a United States Citizen.

Copy of xxxx Applebaum’s 2015 Tax Return, including Form 1040, Schedule C, and Form 2106, his W-2 form for 2015, a Form 1099-INT and a form 1099-R.

Internal Revenue Service Wage and Tax Transcript for 2015.

Support for Part 7 of Form I-864 relating to my assets as the principal sponsored immigrant, including my Bank of America statement (received in the mail) and deposit tickets showing cash assets in excess of xxx xxxxxxx Thousand dollars. ($xxx,xxx.xx). These funds are in my Bank of America savings account, a United States bank.

- Copy of I-797 Notice of Action (Approval of K1 petition dated October 15, 2015)

- I-94 Form: Most recent I-94 and Travel History. (These forms have my most recent maiden name: xxx xxxxx Kim, as they were generated on my entry to the United States on a K1 Visa on May 26, 2016, just prior to my marriage which occurred on June 1, 2016.

- Marriage Certificate: Certified Copy with raised seal and signature of Town Clerk of Agawam, Massachusetts, establishing my marriage to xxxx Applebaum on June 1, 2016. Please note Section 11A of the Marriage Certificate confirming that my surname upon marriage changed from “Kim” to “Applebaum”.

- I-765, Application for Employment Authorization: Completed, signed and dated. Attached thereto in a plastic bag are two Passport sized photographs of myself with my name written on the back of the photographs and with my name on the attached plastic bag.

- Copy of my Passport Biographical Page.

- Copy of my K1 Visa in my Passport

- Copy of entry stamps from my Passport.

- I-94 Form: Most recent I-94 and Travel History. (These forms have my most recent maiden name: xxx xxxxx Kim, as they were generated on my entry to the United States on a K1 Visa on May 26, 2016, just prior to my marriage which occurred on June 1, 2016.

I would include a note stating all vaccinations were received at the K-1 medical and the DS-3025 should be in with the documentation handed over at the POE and sent to the USCIS.

What if I don't have a DS-3025?

If you are positive that you got the needed shots, then actually mailing in a DS-3025 photocopy is not necessary. The original form is with your other medical results and was turned over to USCIS at POE. That's actually the one that counts because it remained in the "chain of custody" so you didn't have an opportunity to alter it. The photocopy isn't really an official copy and is more for a reminder that you got complete at your visa exam. Of course without having a copy, you don't have the opportunity to see if the rest of the form was completed correctly.
