I-693: Documents for Civil Surgeon?

My wife (Ukrainian) and myself (American) recently moved to the US from Ukraine. We're now trying to apply for her AOS.

My wife left the medical exam prior to her K-1 visa interview with a chest X-ray CD and a sealed envelope. When we passed through the POE, the border guard opened the sealed envelope and kept all its contents (we still have the CD, which was kept separately).

Now we're looking for a civil surgeon to help with transcription for the I-693, but I've only now realized that there may have been a mistake along the way. There isn't much for him to transcribe - just data from the chest X-ray, but nothing about vaccinations (she did provide a chart to the doctor in Kyiv, by the way).

Could anyone confirm the documents I should give the civil surgeon here in the US (aside from the I-693) and shed any light on what might have gone wrong along the way?

I don't know how it works in Ukraine, but in the UK you get a copy of the DS-3025 (vaccination record) at the medical.

You got the DS-3025 separately from the sealed, large brown envelope? Strange, we only got the envelope and the chest X-ray CD.

Now we have a real problem, because we don't have records of my wife's vaccinations, and we don't want to administer double vaccinations (can't be healthy, can it?) from a doctor here.
