Tax Return Transcript and I-134 form

I'm the petitioner and i need to get this info to my fiancee in VN; the K-1 instructions state the following

"The original form I-134 must be accompanied by records of the petitioner’s
income for the most recent tax year. The preferred record of income is the Complete Income Tax Return
Transcript from the Internal Revenue Service (IRS) (formerly IRS Form 1722). However, W-2s and a
certified copy of a complete federal tax return (Form 1040), including all pertinent schedules, might be
acceptable in some cases"

I pulled my "Tax Return Transcript" online from the IRS website; the transcripts says "Tax Return Transcript" as the title, but it also says "FORM NUMBER: 1040"

Is this the correct tax transcript to obtain? and is 2015 enough? the instructions only say "most recent tax year"

Second, on the I-134 form, section 1, 1c would apply to me since i became a naturalized citizen through my mom (when i was a minor) and I don't have a Certificate of Citizen number

"If U.S. citizenship was derived by some other method, attach a statement of explanation"

Does anyone have a example of this statement that i can use or see?

for my case:

3 yrs of Tax Return Transcript and 3 yrs of W-2s (2013,2014,2015) you dont need the complete federal tax return since you have the Tax Transcript.

I also submitted 3 yrs of Tax Transcripts along with the last 6 months' paystubs but no W2s. I also submitted a letter from my employer stating my current position, salary, and my start date with the company. Lastly, a letter from my bank (on bank letterhead) stating my current balance, date account opened, and average monthly balance, signed by one of the bank officers.

For your US citizenship statement, I would type up a short statement and have it notarized.

on the I-134 form, if i make over >100K, should i bother filling out the other fields like savings?

recent 3 months paystubs and recent 3 month bank statements are sufficent regarding your case over 100k income
