Benefits Statement enough?

Hi all, me again.

Husband's HR emailed him a summary of his insurance policies (health, life, dental, etc.) that lists me and our daughters as included and/or beneficiary and the effective dates of each. Would this be enough? It doesn't list the policy numbers. I was thinking that I will just ask him to ask HR to print it out on a letterhead and to have the manager sign it but I want to make sure that this will be enough before having them do it.

Or should I submit the actual policy document? We don't really have it, and HR said that she'll have to email the providers for them.

That should be enough for showing you are all included in his insurance policies. If you have any actual paperwork from any of you using the various insurances, such as receipts/paperwork from doctor visits, then include those too.

They just gave him 2 copies of his insurance cards but it only has his name, insurance policy number and employment ID number.

Good idea about the receipts. Problem is that I've never gone to the doctor here yet.

Honestly, we don't have a lot of paperwork evidences to submit but we'll still submit everything we have.

I submitted my benefit statements for 3 years, plus a copy of my insurance card that lists me (the USC) as the holder, and my spouse as a dependent. We also each had a dental claim (different insurance company).

We're still in processing, so I don't know if that worked or not - but I think it will!
