Hello, I am currently completing my i-485 and I have a few questions I hope someone can help with.

  1. Family Name - now that I am married and have taken my husbands name do I use my married last name or my maiden name?
  2. A-Number - I entered on a K1 visa, do I have an A number and if so where would it be?
  3. Current USCIS status - is this K1 or something else?
  4. In What Status did you last enter - is this K1 or fiancée?
  5. Nonimmigrant Visa Number - there is a red number starting with K on the visa in my passport, is this the correct number?
  6. Memberships & Affiliations - what are they looking for here? I don't think I have any but don't want to miss something
  7. Vaccination supplement - was this part of my medical I had done for my K1?

I know this is a lot of questions but I don't want any issues. I appreciate anyone who can help!

okay, so i have figured out my A-Number but still can't figure out the rest. I called the 800 number on the form if you have questions and they were no help at all. everything I asked the answer was 'i'm sorry I can't tell you what to put for that'

I am so frustrated, I hope someone can point me in the right direction.

1) If you changed your name through marriage, then you use your new married name on all the forms and everything else from now on.

2) A# found on NOA2 I-129F approval notice above beneficiary's name

3) K-1 visa holder

4) K-1 visa

5) Yes, lower right, number in red

6) Anything that you have been a member of, exactly as it asks, "..membership in or affiliation with every organization, association, fund, foundation, party, club, society or similar group in the United States or in other places since your 16th birthday" If you have not been a member of anything, then there would be nothing to list.
