My wife is a US citizen and applying for a green card on my behalf.

She is a humanitarian worker and often has to travel and live outside of the US for extended periods of time (6 months - 1 year).

Does it matter if she does not live with me in the US on a permanent basis once the green card has been approved?

No. It can matter when someone gets a two year greencard and needs to establish their relationship in order to get the 10 year card. Typically USCIS would want to see the couple lived together or a lot of explaining why not. But you have been married since 2011 and will get a 10 year card straight off and won't need to prove anything or remove conditions. If you decide to go abroad with her for an extended time then you need to study your requirements for maintaining your status of permanent residence, if you need a rentry permit, etc.
