Request for Evidence - Vaccinations - DS3025 & I-693 for I-485

Hi All,
This is probably obvious to most of you, but I am a little confused.

My wife just received her EAD and AP card, so now she can work and travel :-)

For the I-485, I didn't realize she opted to have her vaccinations done in the US (because she could not find her immunization records). She has been asked to have the DS-3025 (List of immunizations) and 4 pages of the I-693 completed by a Civil Surgeon. So, my question is: What is the definiition of a Civil Surgeon? Is it as simple as making an appointment with a doctor for the vaccinations?

no, you have to find in your city doctors who are authorized by immigration to do the medical and vaccinations

you can check around, since each doctor has different cost

You can find the USCIS authorized civil surgeons in your area from https://my.uscis.gov/findadoctor
