Do you have documentation to establish that you have received vaccinations in accordance with U.S. law?

I have a baby book.

I answered yes because during medical, the hospital will issue us that documentation right?

If your baby book shows that you have received the required vaccinations then it's a yes. If it doesn't it's a no. You need to bring the proof to the medical. If you don't have it or you are missing parts of it they will take a blood test and send it to be analyzed. It takes about a week to get the results. And then they will get back to you and tell you what vaccinations you need to take. In some cases you might only need a booster because some vaccines expire after roughly ten years.

But you could do as I did- I didn't have any proof of what vaccinations I had received during the years and I was missing some but I still answered yes and where you can add notes I wrote "I will receive all required vaccinations during my medical prior to the interview" :)
