Couple questions about two parts in form

Quick question that I don't seem to be able to find an answer for:

In part 2 of the I-601 form, it asks for US Entry information. Would that simply be the last times she entered the US even on just a visit?

Also, under item number 1b, it wants a letter and number that correlates to your status when you entered the US. What exactly is that number and where would I fond that code, if any?

Also, in section 6, it says about information about other relatives with ties to the United States: would that be me again? And the "statement from applicant (discretion)" what exactly is that supposed to be? A statement from her saying why she should be allowed to come to the US even based on her moral turpitude?
