documented vaccinations administered at SLEC

Wife and step kids just finished medical at SLEC. I am unsure so far, as to whether they will, at some point end up with a list of the immunizations they received. We will need some documentation for the public schools in the US. Does anyone know the answer to this?

In with the visa packet (it's what they call the do not open packet) that you will receive when the passport is sent there will be a form DS-3025 that will show all the immunizations received. There is no need for it now so don't be looking for it. It will be given to her at POE when immigration opens her packet.

Just checked and the DS-3025 is with the visa

I was given my own copy of the immunization record after my medical at SLEC. I was told by the person who released my medical packet to keep the immunization record as I will need it during POE. Hopefully there is another copy of that immunization record inside my medical packet.

That was what I thought, but then I asked someone and they said they thought they received it later with the visa package. Whew - my memory isn't gone yet.

I've heard both ways POE and at the conclusion of the medical. Either way you'll get a copy. Just be looking for it.

Give it to you at POE? .. never heard of that one.
