Hi, I just found this site and hope I can get some help. I'm a U.S Citizen and married a Belizean almost 2 years ago. We've been living in Belize all this time but finally decided to move to the States. Right now I have some questions about the G-325A form. Here they are:

1. For the "Family Name" for my spouse, do I place her Maiden name or place her new last name? (It has been legally changed to my last name.)

2. Under "Applicant's last address outside the United States of more than 1 year.", should I place that I've been living in Belize, again? I already placed this information above for the last five years since I'm still currently in Belize.

3. Just to be sure, I'm checking the "Status as Permanent Resident"? I'm filing the I-130 form.

4. Who is the Applicant at the very bottom of the page? The person originally placed at the top of the form or is it my spouse?

First of all you and your wife will need to fill out separate G325A for each.

1. Her new last name. Her maiden name goes on other names used. That's on her G325a

2. Yes you and your wife will put it again on each one's form.

3. You'll check other and put "i-130 for spouse" , your wife can check on "status as perm resident"

4. You are applicant on your form and your wife is applicant on her form.

2015-08-17: I-130 approved

2015-09-23: NVC received file

2015-10-05: NVC assigned Case number, Invoice ID & Beneficiary ID

2016-06-30: DS-261 completed, AOS Fee Paid, WL received

2016-07-05: Received IV invoice, IV Fee Paid

2016-07-06: DS-260 Submitted

2016-07-07: AOS and IV Package mailed

2016-07-08: NVC Scan

2016-08-08: Case Complete

2017-06-30: Interview, approved

2017-07-04: Visa in hand

2017-08-01: Entry to US

Thanks so much for the help arken. Still a little confused on question 2... I forgot to mention I'm doing two separate forms, as you mentioned to do. For my residence for the last five years, I've been living in Belize for 3 of those years to current. I was wondering if I had to place this information again in "Applicant's last address outside the United States of more than 1 year." Other then this, I have not lived outside the US. Seems a little redundant to place it again but seems like that's what they want. Thanks again for the help!

In your case it is redundant but the form isn't tailor written for you. Just answer the questions as asked.

Yes, in your situation, it will be a repetition of information. I found these forms much easier to fill out if you "turn off" your common sense and just answer each question individually;-)

If it isn't difficult, it isn't worth it.

11/4/16: EAD+AP approved (day 53)

11/16/16: EAD status changed to card shipped (day 65)

11/17/16: EAD/AP combo card received (day 66)

12/30/16: Notice of interview scheduled (day 109)

2/1/17: AOS interview (day 142) - APPROVED

2/8/17: GC received (day 150)

 

ROC process:

11/3/2018: ROC window opens
