Hello! Need help! :) do i need to look for a Civil Surgeon.? I got four shots in the Philippines(Tdap,MMR,Varicella and influenza)..

I got four shots in the Philippines(Tdap,MMR,Varicella and influenza)..My DS 3025 doesnt state if my vaccination record is complete or incomplete. Thanks!

No you do not need a civil surgeon as long as AOS is completed within the 1 year validity of your medical. You only need to submit a copy of the DS-3025.

Read page 4, section 7c regarding the K-1 https://www.uscis.gov/sites/default/files/files/form/i-485instr.pdf I suggest you print out this page, then highlight section 7c and attach it to the copy of the DS-3025 when you send it in with your I-485 as some adjudicators at USCIS don't seem to know this. Later if you get interviewed (some do some don't) bring page 4 with you also... saves a lot of unnecessary issues.
