Ok so after the ds160, you need to pay the fee with Banco popular. After 12 hrs or so you will get an email that your payment went through and then you go to the site and schedule your interview

http://www.ustraveldocs.com/do_es That is the site. You will schedule the fingerprinting and the interview. After that you will call the doctors office at the consulate and make an appt. Try to make your appointments on the same day and asap so you can do it at once and prepare for the interview. Make sure your fiancé goes with all the paperwork needed to do the medical exam and fingerprinting. Btw the exam costs $175.
