Hello again to all!

Im currently filling up the Social Securty form and bumped into tricky questions such as:

> should i fill in the "parents/ mothers father's name" in the social security form even if my parents are both in the Philippines and of course doesnt have a social security? Should i leave the area blank?

> Can i use my Full Maiden name in the "other names used and Full name at birth"? (already married now).

Yes fill in parents names, of course the SS# part is blank ;)

Yes use your name on your birth certificate for other name used.

- be prepared for the SSA agent to issue the card in your maiden name, even though you have your certified copy of your marriage certificate, some agent go by the name in the SAVE database and that is it. My wife didn't have an issue getting it in her married name.

should i type my married name already in the "Name to be shown on card"?

then, full name at birth is also same as my other names used (such that it is my full maiden name??)

im double crossing my fingers it wont have any issue getting it in my married name.

I would, that is what we did with my wife, we used her married name (traditional Philippine format).

If they issue it in your maiden name don't stress on it. Once you complete AOS - Using your married name on all documents you can stop by SSA and get your card updated, need to do that anyway to remove the NOT FOR WORK statement.

State requirement vary by state, so can't help you there, need to go on your state's website.

With my wife we applied for the EAD I-765 and AP I-131 when we filed the I-485 for AOS, the EAD combo card arrived a little over two months after filing (in her married name) we took that to the state to get her driver's permit (ID), a few months later her green card arrived.

so we really have to apply for EAD with AOS all at the same time...

i read in Florida State ID requirements, i have to present a social security and an EAD so i can get one.

Does the electronic I-94 has to be notarized or something?? Or it's okay to submit it fresh from the printer?

Can i change and use my signature (in my married name) in the Social Security form? It still confuses me because, they might compare my signature in my passport (maiden signature) with my new signature (now that im married).

thanks a lot!!!!!!!!!!!!!!!!!!!!!!!

You do not have to apply for EAD I-765, it is optional, and also free, when applying with the I-485

I-94 does not need to notarized, just print out a copy

Yes, use your married signature on EVERYTHING! You are married... that is a fact of life.. signature changes SSA can't force you to use a different signature ;)
