i-130 For Father who had lived in US for 15 years

Hello,

I am a US Citizen and filling the 1-130 for my father who had lived in the US for 15 years. He left the country since 2005 and is 10 years bar is up. Do i need to fill a waiver for him?

What else do i need to include on the application?

I am including:
Form i-130
Copy of naturalization certificate + copy of passport
copy of my birth certificate showing the names of both parents
Parents marriage certificate (Mother is deceased- should i include death certificate?)
Check or Money order for $420 (U.S. DEPARTMENT OF HOMELAND SECURITY)
Form I-864: Affidavit of Support (Do i need to fill this and how much do i have to pay?)
