I am about to apply for adjustment of status from a K-1. I have everything I need except the medical vaccination supliment (I693-A).

I read the details here and it says simply take your DS-3025 vaccination report you received at your K-1 medical to a US civil surgeon and they will fill out the required suppliment so you can send it in with your AOS application.

I didn't receive a DS-3025A from the Doctor in London. My medical exam results were sent direct to the embassy for my interview and all I took away from the Docs was a photocopy of my vaccination record from my own doctor and a cover letter stating to my doctor that I was required to have 2 vaccinations for my residency in the US. The doctor in london told me I wasn't required to have these to travel to the US on my K-1 and could simply have them done when I arrived stateside before adjusting status.

I have found a local civil surgeon, so if I go to him with the photocopy of my vaccination record and my letter from the London doctor, and have the two required vaccinations; will I be able to get from him the vaccination suppliment required for the AOS? Or should I simply send in the stuff I got from the london doctor with my AOS and wait and see what they say?

Oh and by the way, I don't require a full medical as it has been less than a year since this was done.

Help would be good, thanks.

I've heard of a few cases where people have sent in the DS-3025 in lieu of the I-693A and not gotten an RFE. But I've never heard of anyone just sending in a record of vaccinations. Safest thing will be to get the civil surgeon to transfer. The one we went to didn't have a problem filling it out using a regular old vaccination history since we didn't have a DS-3025 either.

ETA: I would confirm by phone when you make the appt with the civil surgeon that he can transfer from the record that you have.
