I'm getting the medical done a couple of days before the official marriage ceremony. I'm currently in the US on VWP we didn't intend to marry until a couple of weeks ago and been rushing to get everything sorted out before the ESTA expires. My question is whether I put my soon to be married last name on the form or my current last name? Since there will be a date on when the medical was completed on the form I'm worried that using my married last name would be considered inaccurate or untruthful but I'm also worried it wouldn't be consistent? Has anyone done this and what last name did you use on the form?

2013 - Met online
2014 - First met in real life
2015 - First time living together for extended periods of time.
2016 Early May - Got married
2016 Mid May - Send off AOS application (Waiting for response)

Hi and welcome!

First of all, no need to rush everything. Your ESTA overstay will be forgiven due to your AOS based on marriage.

Always use your current name (so on the medical before the wedding, use your maiden name). You will show proof of maiden and married name on the AOS paperwork, and I'm sure immigration will be able to connect the dots.

If it isn't difficult, it isn't worth it.

9/14-18/16: Received 3xpaper NOAs 

9/23/16: Received biometrics appointment letter (day 11)

10/3/16: Biometrics appointment (day 19)

11/4/16: EAD+AP approved (day 53)

11/16/16: EAD status changed to card shipped (day 65)

11/17/16: EAD/AP combo card received (day 66)

12/30/16: Notice of interview scheduled (day 109)

2/1/17: AOS interview (day 142) - APPROVED

2/8/17: GC received (day 150)

11/3/2018: ROC window opens

Thank you! I thought that would be the case but I just wanted to be crystal clear about it.
