Preparing the AOS Packet- Don't need evidence of Medical Examination?

Hi,

After reading this and the instructions:

I-693, Medical Examination of Aliens Seeking Adjustment of Status.I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. If you are missing any shots from your overseas medical a I-693 vaccination transcription is required to be completed by seeing a Civil Surgeon to have certain portions of the I-693 completed and have the Civil Surgeon certify the form (sealing the original in an envelope and giving you a copy).

My husband and I are still confused as to if we need to provide some kind of evidence that he recently had his medical exam in Madrid, or does having a K-1 Visa evidence enough?

Thank you, everyone!

We never sent anything related to the medical with our I-485 because we never got a copy from the medical exam. We were hoping it was in the package that was turned in at the airport and would be matched up with our file. It must have been because we had our interview last fall and my husband got his green card. They never asked a thing about it.

The original DS-3025 is included in the sealed envelope along with the rest of the medical exam documents that is handed over at your POE. There is no requirement to include a copy of your DS-3025. It is just typically included to highlight and point out the fact that the applicant has met the AOS vaccination requirements. The pinned thread in this forum by Nich-Nick actually touches on the topic of not having the DS-3025.

Thanks! I don't think he received one! Does the form have a name or number?

Yes, it is the DS-3025. Read this thread > http://www.visajourney.com/forums/topic/551080-immunizations-ds-3025-or-i-693/

Great thanks then i don't need to think of that one. Our Doctor send us the vaccinations papers after the medical. But I guess I don't need to send them in. Thanks for clearing that out! :)

You probably should at least point out on the cover letter that all required vaccinations were obtained and the DS-3025 should be in his file at the USCIS. If you have a DS-3025, then why not send in a copy? Sometimes pointing out things and 'holding their hands' is what you need to do to get by.

It all depends on the person looking over the AOS package. My husband had his K1 medical in Jamaica in Dec 2015 and we got out NOA for AOS on 3/17/16 then a month later got a notice to applicant letter in the mail stating we hadn't included a medical form (we did include the vax form) with his package. So now they want him to do the medical exam again. No consistency. Good luck. I am sure it was just a fluke for us.
