I am going to try to ask for help again - I want to start by saying I have read the Instructions for the form multiple times. I am hoping for someone to help clarify what to do in these specific situations...

Regarding leaving things blank/putting N/A...

Am I actually supposed to put N/A for everything that is truly not applicable? For example: A-number (for me as the US Citizen), for Physical Address Section (same as our mailing) am I supposed to put N/A for everything INCLUDING the state abbreviation? That seems confusing...also the whole children section, what do you do about the YES/NO check marks? Signature areas for Interpreter/Preparer?

Our entire package is ready to be sent out aside from this form.

I leave things blank. For the A- number (I'm the US citizen and left blank). Are you using the correct form? There is a new form that just came out recently. For physical and mailing address you check the box if they are the same. Left interpreter and preparer entries blank as well.


Just make sure you include those pages for the the preparer and interpreter. You have to submit the complete document even if those are blank. We also left the children section blank as well.

Filled AOS/AP/EAD: 2014-04-09

Biometrics: 2014-05-08

AOS approved for interview: 2014-06-24

AOS Interview: 2014-07-30, Approved.

GC received: 2014-08-11
ROC Sent: 2016-04-29

ROC packet received at California Service Center: 05-02-16

NOA1 (Received 05-13-16): Letter dated 05-02-16
ROC Biometrics: 2016-06-02

Filed for Re-entry permit: 2016-06-03

Re-entry permit biometrics: 2016-07-14

Re-entry permit approved: 2016-12-07
Re-entry permit received: 2016-12-10, Only got 6 month re-entry permit????

Leave the fields that don't apply blank. Also, why are you filing out the I-751 of you are a US citizen?
