Need advice please. For the G-325A and AOS forms, I understand my wife can use my last name now. Can she also change her middle name too on all forms? Thanks in advance.

She can only use your last name if it was officially changed. She can't just use your last time b/c you got marry. So it would be the same for her middle name was it officially changed? Also you will need prove that her name was officially changed.

This is incorrect. She can use his name just because they got married. That is how name change through marriage works. The marriage certificate is the legal name change document, even if only her maiden name appears on it. There is no other proof of the name being changed 'officially'. It is official when they marry.

Yes she can.

My wife is a Filipina, and it's common for the middle name to change to the maiden name while taking the husband's last name.
