Timeline

I have DS3025 with me which the physician from Pakistan signed and stamped etc when I had my medical done there. Is this all I need to take with me at the interview or do I need to get some kind of medical report from US physician as well?

I'm assuming they'll let you know what you need to bring with you for the interview? Did you submit the DS3025 in your AOS packet?

I also have the DS3025 with me and I don't plan on visiting a U.S. physician unless the USCIS specify they need me to do so however I've seen people do both - either they just bring the document from the K1 medical or they get it updated by a U.S. physician just to be on the safe side.

Actually in the letter they have said to bring a completed medical examination (Form i-693) and vaccination supplement in a sealed envelope. So I do need to go to US physician I believe?

The doctor that I have talked to here as plainly refused to transcribe form i693 for me and says that he will perform a full medical exam and then fill it out for a fee of $295 + any lab work fee. This sounds utterly ridiculous to me.

This is your appointment letter? It has a generic thing for everybody that says if you have not submitted a medical report, bring it to the interview. Well hopefully they do have your visa medical and shot record in your file to evaluate. Sometimes they lose that.

I personally would wait, go to the interview and see if they approve you on your visa medical. Then you are not guessing what you need. The officer will tell you face to face if it's whole medical, shots, or nothing more that you need. Then you go get it done only if required. Mail it to the interviewer and get your card issued shortly after.

In my DS3025 only Polio IPV is ticked and dated with class B. I was not given any other shots, nor was I told that I would need them in US. My lungs Xray for TB was done as well and I have its CD with me. At the bottom of the form for the summary it states: US Vaccination Requirements COMPLETE (Requesting a Blanket Waiver)

So it still means I don't need any other shots?

There are four shots for adults that need to be on your DS-3025 or waived. Polio so not an adult shot.

Varicella can be waived if you had chickenpox.

Infuenza can be waived of it isn't flu season. Both would have to be marked on your DS-3025.

You also need a date for Tdap or Td within the last 10 years.

You need MMR. If the only thing on your sheet is IPV, then you need shots and a new immunization sheet filled out by a civil surgeon before your AOS will be granted. You didn't need shots for the visa but you do for a greencard.

Does anyone know of any civil surgeon who would be willing to give me a signed and sealed i693 with the updated vacc info if I get the vaccinations from my local doctor? That way I can pay for vacc through my insurance and pay the civil surgeon the fee for the transcript.

You can get shots anywhere....your doctor, a drug store, a health department, or a civil surgeon. Insurance doesn't always cover vaccinations. Check first. Any civil surgeon can copy them to the I-693. The problem is finding one who doesn't insist you have a new exam. Call around and ask.
