This is very confusing. When checking ceac site it says our case is ready for us to schedule an interview date. But we just emailed packet 3 and haven't received packet 4 yet. So is it a new procedure in montreal to schedule your interview date first and wait for packet 4 later?

No. Email them again about packet 3. Do not schedule an interview without packet 4. You run the risk of them cancelling an interview without notice. Plus you need packet 4 for both the medical and to get into the consulate.
