K1 visa medical question. Just send the DS-3025 or also fill in I-693?

From what i understood from this topic http://www.visajourney.com/forums/topic/551080-immunizations-ds-3025-or-i-693/ i'm not required to send in the i-693 if i enter with a k1 visa. I was wondering if i still need to send in the i-639 partially filled in or just not send it at all? I've read some people have been getting RFE for sending in a partially filled in i-693. Do i just send the GS-3025?

Thanks

It is one or the other. If you do not need to see a civil surgeon for an I-693, then you do not. Follow that thread and the instructions. Send a copy of your DS-3025. Only an I-693 filled out and signed by a civil surgeon should ever be sent. Never partially fill one out on your own.

Ok thanks :) Just the DS-3025 it is then. Do i also add the receipts that show I got the medical done in my home country along with it?

I only sent the DS-3025, Nothing else medical related.

The white sealed medical envelope I got back from the Amsterdam consulate, is still in my possession. I figured that if they want it, the would ask for it and if needed I would hand it over at the interview (but I was approved without an interview)

No, just a copy of the DS-3025. They have all your medical documents. They were in your sealed envelope you handed over at your POE. The copy of the DS-3025 is basically just to highlight/point out the fact you had all the required vaccinations for AOS.

Nope, they only want the big yellow one. The white one is for the AOS part, but as I stated, I didn't use it.
