Has any one made Affidavit of birth here?

Hi

I am from Nepal and

I dont ve birth certificate. And i have heard that affidavit of birth from parents and close relatives ll also work in the interview. But i dont where i can make it? Do i need to make in muncipility/vdc or cdo officer or where? If anybody have made it, please provide me the sample.Thank u in advance☺

No body respond in this post . Eventhough I am going to share my experience how I had to go through. This post might be helpful to someone in the future.

I didnt have BC so I made the janma darta sifaris (affisavit of birth) from VDC secretary of my birth place. In Nepal, Embassy doesn't accept any kinda affidavit of birth written by the parents or family members. It should be issued from the government officals. There was one guy at the interview infront of me who had Birth certificate issued only from the Teaching Hospital, was asked to bring one from the VDC /muncipility. He thought as Teaching hospital is also government hospital so BC issued from there can work but it didnt.

BC could be in Birth certificate format which written in both Nepali and English or sifaris(recommandation of birth) hand written in vdc/muncipility letter pad in Nepali language(need to translate in English).
