Is it necessary to include Form I-639 in the AOS packet? I had my medical examination last November and we're about to submit my AOS package soon. I'm a little confused 'coz my DS 3025 seems incomplete. I don't know if the vaccination record that i have is USCIS approved. Also, just to confirm, i have to fill some parts of the Form I-693 and take it to a Civil Surgeon for completion and his signature? Correct me if I'm wrong.

I attached a copy of my DS 3025. if anyone knows if my Vaccination record is complete and correct, please let me know or if it's alright to submit it along with the AOS packet.

Thanks and hoping for everyone's opinion.

You are missing the Tdap. You will need to get that vaccination, then see a civil surgeon to get your vaccinations transcribed onto a I-693 form.

If i get a vaccination from the local health department, will they give me a new DS3025 or are they just gonna sign on my old one? Please help me through this process. We are contemplating about this stuff 'coz we want to submit our AOS packet soon as my I-94 will expire in less than a week.

Must be done by a civil surgeon. Btw it is okay if your I94 expires before you send in the AOS. Call around to the civil surgeons to see if you can find one that will just charge for the immunization a and not the whole exam.

The health department will not amend your DS-3025. That form is given by a panel physician doing immigration medicals abroad. That form is done. There is no adding to it. They will provide you with a record of their own that they gave you the vaccination. You will take that record and your DS-3025 with you to the civil surgeon.

The pinned topic in this forum lists what a correctly filled out DS-3025 needs. If you are missing one of the required vaccinations, the Tdap, then you will need to get that and have a civil surgeon transcribe your vaccinations onto an I-693, as you were told in the first reply of this topic.

Sorry scanner broke, but I just looked at mine again and yours does not look to be complete.

Item 1:

All vaccines are either marked with a letter in the last column or with a date of when you got them.

Item 2:

The second tick box for the waiver is marked.

Front is signed and dated by the physician and the back has the initials (and date).

That was the only medical form I included in my AOS package and I got no RFE's.

You will need to get the remaining vaccination at any pharmacy like walgreens, kroger walmart etc but you will need to keep the receipt from them and take them to a USCIS civil surgeon and show him you DS-3025 along with your receipts for other vaccinations done in the US and he will transfer those details onto I-693 and make sure he includes a copy of your DS-3025 into the sealed envelope. You should be able to find a USCIS CS in your area who will charge around $35-$40 just to complete the I-693...Check here to find one at your local
