Hi again! My fiance didn't get any vaccinations for the K-1 visa while in CDJ. Hence we have NO vaccination record at all from the interview process. There is a civil surgeon very close to our house so we weren't worried about it. Until......we made the appointment and they asked us to bring the "form that has all his vaccinations listed, that's very important". I assume they mean the DS-3025-but what ever they mean we don't have one. We have nothing from the medical except the CD which I think is the chest X-ray.

Is there any rule or procedure I can print and bring to them about what they should do in this case? I think in the past all the K-1 people who came to that doctor's office had the DS-3025-so they assume everyone has it. Surely there is a way around this. Anyone know what to do?

Yeah, I wish CDJ wasn't so lazy about vaccinations for a K-1. This causes problems when it is time to file for AOS. I know you do not have any from the K-1 exam, but do you have any vaccination records? If so, take those. If not, then go without. The civil surgeon just needs to determine which vaccinations you need. MMR, Tdap/TD, and varicella(unless you already had chickenpox) are required for AOS. You won't need a flu shot since it is no longer flu season.
