How to report money wire transfer?

I have read many posts stating that transfers of $10,000+ USD (even if done incrementally) must be "reported". How does one report it exactly? Is there a specific form?

The money is my foreign wife's personal savings. It's going from her Netherlands account to her USA account.

I might be mistaken but I think it is the bank that reports the transaction to the regulatory authority, not you. When we moved to the U.S, we transferred far more than $10 000 and had to fill out additional paperwork at the bank in South Africa, verifying the source of the funds. Not sure if that's just a South African bank thing. Anyway, our U.S bank received the funds and did not ask for anything more from us.

You only have to personally do paperwork when you bring $10k+ in your pocket on the plane. Otherwise the paperwork is handled by the banks or transfer agency.
