Q 10 my citizenship was acquired through birth in the U.S., I marked that. Do I need to enter anything in this box "Give number of certificate, date and place it was issued."?

No, this is for naturalized citizens.

Okay, thanks. one more question: I accidently wrote the wrong information in Q 12, can you just put a line through it, and write N/A (the right response), or do I have to start over?

No if you were born here, you do not have a naturalization certificate - that's all you need to fill out there.

The rest does not apply to you (from what you have said).

Having worked for the Government and filled out thousands of forms, you can make the aforementioned correction and put your initials next to it.

Line Out or Not... I would start over.

You can download the PDF file and type it in.

Why? Do you think the Government is that #######? They are not.

Personally, I used the PDF option, but I could not save the filled out documents.

After noticing a minor mistake, I did exactly what I advised above. Result: Approved.

Ok far as corrections what about the 156 i printed on the computer sent to him but then we found he had to extend his passport> but instead of just extending they issued a new number, so where i had put his number and that date now need to be changed to the current one. Line out neatly or just let him re write another form

Just write: (Passport renewed and add the new number). USCIS will appreciate the old passport number, along with the new one. In fact, they may issue an RFE (Request for Evidence) for the old number, if you did not provide it.

Good point, let me clarify by saying that the applicable Consular Section will appreciate getting all of the relevant passport information.
