I 693 confused!!

We once again need the expertise of the VJ people. We were calling around to see how much it cost to transcribe the DS 3025 to the I 693, but to my surprise nobody wants to JUST transcribe instead they want me to do another full medical examination. I understand that I don't have to do the medical again since it has not been a year. My DS 3025 is not marked complete though because they only gave me the MMR and Varicella vaccination at St.Lukes. Can I get the other vaccinations from our family doctor so that I can comply with the requirements and just send the vaccination record(DS 3025) for my AOS without going to the civil surgeon? Or is that too much of a risk for denial? Thank you all very much, we are once again lost in the process?

The report must be from a civil surgeon. Do some searching around. Some of them (civil surgeons) won't sign off on anything that they haven't done. Others will transcribe from the vaccine report or another doctor.

I-693 is not needed. Transcribe is not required. A copy of your vaccination record (DS-3025) is listed as required.

READ the instructions for the I-485, page 4, section 7c regarding the K-1 visa. Then print out page 4, highlight section 7c and attach it to a copy of the DS-3025.

My wife's DS-3025 was marked incomplete also, no issues. You can get the missing vaccinations at the county health clinic or your doctor if you desire.

Thank you Hank! Yes we will get my vaccination at our family doctor. My question now is, what proof do we send to USCIS that I got the vaccinations required? Or does the doctor fill out and mark my DS 3025?

Your doctor has a vaccination record form (booklet), ask for that. Do make sure to print a copy of page 4 of the I-485 instructions and highlight section 7c - some adjudicators at USCIS don't seem to have a clue how to process a K-1 AOS

I-693 is not needed. Transcribe is not required. A copy of your vaccination record (DS-3025) is listed as required.

READ the instructions for the I-485, page 4, section 7c regarding the K-1 visa. Then print out page 4, highlight section 7c and attach it to a copy of the DS-3025.

I guess you could complete your vaccination at your doctor, not sure what form he will us to record the vaccination, but just make a copy of it and include with the copy of the DS-3025. We sent in the DS-3025 with the incomplete vaccination and had no issue.

And don't forget to print out a copy of page 4 of the I-485 instructions, highlight section 7c and attach to the DS-3025.

Did you not complete your Vaccinations at SLEC during your medical? If not and it was because they did not have the vaccines then you are eligible to return to complete at no charge. All vaccines are included in your medical fee.

I was given three shots in SLEC: MMR, Tdap, Varicella. There's no remark in my 3025 if it's completed nor incomplete.

But you said Hank that it got no issue when you submitted 3025 with incomplete vaccine provided if will attach I-425 page 4 instruction.

I am plannig to have it completed at my local hospital here in Phils, but i dont think if US will honor the record....

Look at your DS-3025. If it has the ALL of the following items, you don't need an I-693 (Adults 19-49 yrs. old).

1. Shots marked with a date (at least one of a series)

MMR (two are required but if you have "insufficient time interval" checked, then you get a waiver)

Td or Tdap or DT or DTP or DtaP (One no longer than 10 years ago, i.e.. have a booster)

Varicella or VH written by it if you had chickenpox

Influenza (during flu season)** see note below because you are most likely okay without it.

2. Everything else marked not age appropriate, insufficient time interval, not flu season...things that allow a waiver.

3. RESULTS section filled in with "incomplete" and "may be eligible for blanket waiver" ticked or "complete" (but complete is rare that it is ticked). So if it is blank then you are good to go without any additional vaccinations needed.

4. Signed and dated by the doctor

If it's not completed EXACTLY like that then see a civil surgeon for the I-693. If it is complete, send a photocopy of your DS-3025. Almost isn't good enough. It needs to be filled out properly.

From what you are telling us you seem to have had all your shots and there is no need for additional vaccinations. Read above and compare with your 3025.

In my case they gave me MMR and Varicella at St. Lukes, my DS 3025 is not marked too but basing it from what you're saying Greenbaum, it is incomplete. Our plan now is talk to my OB for the allowed vaccinations since I am 10 weeks preggy and get it at the local pharmacy or our family doctor. Get a record of that vaccination, attach it to the DS 3025. Do you think it'll work? We just can't find any Civil surgeon in the area who is willing to transcribe without giving me a full medical exam.
