I agree, you can request an extension and a new medical, it won't hurt. But keep in mind they consulate will ask for a VALID reason for the extension and the fact that she would "rather not travel alone" is not a valid reason. Others from her country who waited literally years and years to get their visas who would have rather not have traveled alone did so, and they lived to tell about it. In fact I can guarantee you that if you were to poll every person who had ever traveled here alone to ask if they would have rather had someone to travel with, 99% of them would say heck no. When my husband had his interview in Amman I traveled there so if he was approved on the spot, we could come back to the US together, that didn't happen, he had 10 weeks of AP. The day he was approved he booked his reservation ans was here 5 days later, he traveled alone. Did he want to? No, but he did what he had to do to order to get here. So don't be surprised when the answer fro the consulate is no. In the mean time she is getting closer and closer to the time she needs to travel. Before my husband received his visa I traveled alone to Jordan by myself 6 times and I had never set foot out of the US before that, I left my 2 children behind in the US in order to do so. Trust me it was not an easy thing to do. If your wife truly wants to get here, she will have to conquer her fears and get on the plane.

Always an option to pay for airport attendant for her. They can get her in a wheelchair and take her to the airplane, same on connecting flights and all the way through POE. Lots of people do this for their older parents who don't speak much English.
