Health ins and tax question, No joint bank account (merged)

Hi

I am in process of collecting the documents for removing the conditional residency documents for my wife

We are getting family health insurance through my employer. We both have the insurance card for that. She is covered with me as family member since she arrived US.

What do we need to submit to USCIS to prove that we both are under my insurance (through my company) since she came.

I can provide copy of the cards (my and her) but that does not say from which date it started.

What else I need to collect?

Do I need to ask insurance company provide some letter?

A copy of the card should be enough. You could also print out the page of the policy detailing who is covered.

If you can call your employer's health insurance provider, ask them if you can have a copy since you added you wife as dependent or beneficiary OR if you can just print out the previous&current enrollment for your benefits (if applicable to you). Providing copy of your cards will suffice, but much better if it can prove and cover for 2 years (of being married).

IRS tax transcripts are higher quality evidence than the returns as they show that taxes have actually been filed. You can request them online and you should receive them in 5-10 days. I received mine in less than a week. Here is the link:

Great point. I requested my 2015 transcript a few weeks after filing in anticipation of ROC.

We did not have joint account. Conditional CG will expire in Dec 2016. So we need to file I-751 in september.

I did not know that we need to have joint ac statements/ Credit card account / lease etc.

I am planning to add my wife to my CC and bank account within next 2 days.

Our Situation:

- Same address on her State ID and My Driving Licence

- Both are covered through my employer health care plan in current year and precious year

- I just added her as the beneficiary on my 401k (Just added 2 days back).

- We are going to have baby in may.

- We do not have Joint statements of bank/lease/property till now.

- We have separate bank accounts (Not Joint) but the address is same on my statement and her statement

Is this sufficient. What else we should try to collect?

So we need to submit ONLY Transcript we received from them

OR

We need to submit Transcript and the copy of all the documents I got from HR Block?

Transcript only.

The reason tax returns are not usually acceptable is having a tax return shows you prepared the documents, but no record of filing unless you provide that proof. Tax transcripts document you have filed, and it's been accepted by the IRS. Ex: I have my tax return, ready to submit, but have not done so just yet.

It's good to know you have known about this, still not too late

- Notarized affidavits of 2 or more witnesses that you have lived in same address you knows both of you e.g. parents/family or friends, collegues, et al

- pictures with her/family et al

- receipts or any proof if your travel tickets, movie tickets et al

- car insurance (you can actually add her even she is not driving)

- any credit card and include her as added/authorized user

- you can also add her in the property/lease, car ownership

- how about utility bills? mobile phone, electric, internet et al

- gym or any membership

- anything that will show you have been together and making some financial co-mingling

**** Also, you do not need to create different threads for your questions. Just combine everything. Good luck on you ROC, you still have plenty of time.

I'll be filing for ROC soon. Is there an actual USCIS form for affidavit? Or just a letter? I plan to have my doctor and dentist submit affadavits, since we both go to the other's appointments. My dentist seems to think there is an actual form (she herself is an immigrant), but I can't find one.

There's no form - it's just a letter, notarized if possible but not required.
