My husband just went to his 10:00 am, and even though they wrote VH for varicella, which means he's immune, the civil surgeon tried to get him to pay an extra $150, etc. for bloodwork. So, I told him to leave. Does anyone in the LA area recommend a good civil surgeon for the I-693a transfer from DS-3025, who won't extort money from you? We're in the West LA area, but willing to drive if it saves us $100!!!

We were quoted all kinds of different amounts to get the I-693A completed. I think most of the civil surgeons are just out to rip people off, in my opinion.

I was glad that we found one in the Seattle area that did the I-693A for $35.00, with no vaccinations required.

Most of the other ones I called insisted that another medical exam was necessary, even though it hadn't been more than one year from the last medical exam.

Good luck finding a good civil surgeon in your area.

I found a civil surgeon in Van Nuys who charged me USD 40 to transfer my DS-3025 to a I-693A form, and another USD 20 for a TD shot.

Oh, that's not bad. Which one was that? Do you have his/her contact info?

We went to this fellow and he was very reasonable:

He did want to charge us for xrays so I went home and got the one Claudeth had from her physical.

Do you still have the form from the medical exam, I think it is the DS3025 but can't remember. That was very helpfule to the doctor I took Claudeth to.

I sometimes think that being designated a civil surgeon by the government gives them a right to 'steal' :angry:

Hey guys,

I would be very very VERY grateful if someone could suggest a civil surgeon in Los Angeles. I have been scoruing the list on USCIS and calling practically every one but all of them want to charge for a full physical, bloodwork and vaccinations. I'm getting so frustrated! I'm willing to pay up to $100 but I just want someone who knws what they are doing!!

Hey Buffertortoise. I haven't used him but there are many here who have done so successfully. If you would like to see what exactly he is filling out, just request a copy for your records. I didn't send in an I-693A with my AOS submission because my vaccinations were completed as part of my medical.

After much searching in the LA area myself for one that would do just the 693A I found this one:

When I first called they said all the same bullcrap about $100 for blood work and stuff, but when I brought in the page of the I485 information that states for a K-1 visa you only need the 693A, all of the sudden it was $30 and come back in one hour for the signed form. Easy as that.

My Timeline

K-1 Process:

3/30/07 - NOA 1

6/25/07 - NOA 2

8/30/07 - Interview

9/13/07 - US arrival date

AOS Process:

10/5/07 - Married

10/19/07 - AOS application sent (I-485, I-131, I-765)

3/7/08 - Interview - APPROVED

3/17/08 - Greencard arrives

Removal of Conditions:

01/12/10 - Removal of Conditions package sent

01/19/10 - NOA 1

02/11/10 - Biometrics

03/16/10 - Approved

KRIKIT, IS RIGHT ON, use this one I know from personal experience, only $10 fee and you can do it all by mail just call leave message and they call you back and then tell them what you want and they will tell you what do send to them. No hassles or Dr trying to rip you off.

Dr Arnold is great. The ladies who answer the ph (and yes please ph and chat to them first). You might have to leave a message on their machine but they do ph back quickly, are very well informed on what is needed for the supplement.

You will just need to send proof of the vaccinations, either the worksheet or vaccination records and they will do it all and send a copy for your records with the sealed one for USCIS.

Hey guys,

So I continued calling around to try and get an appointment with a Civil Surgeon in L.A. who would be able to fill out the I-693A for me without a medical. After being told by virtually everyone that I would need $250 medicals I managed to find one place who would change $20 and do it while I waited! They said to come immediately...little did I know that the office is in an EXTREMELY bad area! Anyway myself and my wife decided to give it a shot.

The doctor filled out the form once and did it incorrectly so I asked her to do it for me again which she was not best pleased about. She filled it out a second time and sealed it and gave me a copy to read over. Everything looked fine and I was so relieved to leave the scary place that we left. However I have now got home and found a few errors on the form and am just wondering whether I should send it in as is or go back for another fun day of waiting around.

Basically the errors on the I-693A are:

The doctor did not fill in my passport number, this has just been left blank

The doctor has added an additional date to my Td vaccinations, stating I have had 2 vaccinations when I have only had one

The doctor has checked the box marked "Vaccine History complete for each vaccine, all requirements met" even though there are several vaccines that I have not had as they are "not appropriate age" and on my form from my UK medical DS-3025 the doctor checked "Applicant may be eligible for blanket waiver(s) as indicated above." Should the civil surgeon have checked this box also?

Do these seem like severe problems or something that USCIS will not even care about? I am planning on sending both my I-693A and my DS-3025 with my application.

I would really rather not take another trip to the doctor if possible!
