On page 2 you would list your parent(s) IF they are dependents, and you had filed affidavits of support for them (but only list them once, if you filed affidavits before then list them there). Your fiancee is listed on page one, you don't list her again on page 2. Item 11 you can ignore. After that you sign and date the form, ignore the blank space in the OATH.

Your income must meet the 125% requirement of the I-864P for the number of people listed on the form (including you)

#8 put my parents names because I filled as dependents in my tax 2015?right or wrong?

#9 put parents names because I filled affidavit of support last year dec 2015..?right or wrong? Where could I find the date filled?? Is it when I signed the paper??
