My husband has his appointment (for $25 hopefully) to switch his DS 3025 over at 10. Is there a blank copy of an I-693a out there? Checked the USCIS website and no luck.

Why do you need one? If you're going to a civil surgeon to have this information transferred, the Doctor will already have this form.

:thumbs: I hope they don't blind side you with all sorts of fees. We called around and found a huge spread in the fees and ended up paying $70 for the supplemental in a sealed envelope and also a copy.

Ours filled it out for the $40 cost of the one vaccination my wife needed. And my FSA reimbursed me, how sweet is that!
