Submit Inaccurate DS-3025, or Get Corrected via I-693? [split topic]

Hi everyone -

I have a quick question regarding DS-3025. I was assisting my wife filling AOS and found out that her doctor forgot to put "VH" on Varicella History back in her country even the doctor had asked her if she ever had it before. Do you think it is okay to submit a copy of DS-3025 or should we go to local civil surgeon to have it corrected with I-693?

I also attached the DS-3025 if someone can help me to verify.

This may already be asked but I'm curious also about the DS3025. My fiance had his medical overseas before he came and was given an envelope after the medical..when he arrived he gave the Immigration officer the envelope from the embassy and the medical so we don't have the DS3025 form..will this be an issue??

The doctor did not put that there was a history of chickenpox. They used code D, which is that the vaccine is not available there. You still need the vaccine or a civil surgeon to verify and mark off VH history on an I-693. http://www.visajourney.com/forums/topic/551080-immunizations-ds-3025-or-i-693/
