Medical with Dr Lyndon Mascarenhas

Hello everyone,

I am about to have my medical with Dr Mascarenhas on April 4th. I have gotten my doctor to fill out and sign the vaccine checklist that is on Dr Mascarenhas website. I am wondering if that is enough proof for my vaccinations or I have to go back to my doctor's and get some formal documentation?

Sorry if this was already asked - I tried searching on google.

You should have documentation showing when you had the vaccinations or that you have proof of immunity.

Generally available from the health clinic or your medical records. Or your doctor can order tests to determine immunity.

I do have a vaccination record from public health but it is not up to date (only up until 2005). I actually just got my tDap booster in February at my family doctor, where I also got her to sign the checklist showing the dates I got the required shots for tDap (February), and MMR. I left the office without any documentation of getting the tDap booster, other than the checklist. So, I guess I should go back and get her to stamp one of those yellow cards showing the dates I got those shots?

I did my medical with Dr Mascarenhas a couple of weeks ago. As long as you filled in the dates receiving those vaccinations, that's fine. Just the completed checklist is required, they did not ask me for any other formal documentation. The whole process was very easy too; the doctor was so busy that he only asked me three questions in all "What's your height? Weight? Have you had any serious decease?" And then checked my blood pressure, and sent me to the x-ray and lab! No worries
