Forms I-693 and I-94 question

My fiance finally came to the US and we married. Now it's time for AOS, another ridiculous process :(

This is regarding form I-693. It says she does not need to have another medical examination if the I-485 is filed within 1 year of the one she had abroad. It also says she needs to complete certain portions of this form and have a civil surgeon certify the form. What part of this form needs to be filled and where do we find this civil surgeon? Could you clarify exactly what we need to do as I am very confused...

Also when she entered the US, they put her name in the wrong order on her I-94. They put her middle name as her first name and first name as middle name because of how it is written in her country passport because of different standards. Can she apply for AOS with this form and write a note that they made a mistake or she needs to contact border security to change her name first and then apply for AOS?

Timeline

She needs to have the vaccine portion completed by a civil surgeon (last page of the form).

You can locate a civil surgeon certified by USCIS by going here: https://my.uscis.gov/findadoctor

It may or may not be an issue but, she should go to a CBP office and speak to them about getting it corrected. It will most likely be an issue when applying for a SSN.

Isn't this only if the foreign spouse did not have a complete vaccination record or get all the immunizations required for their age during the medical done at the foreign panel physician? If the records of the vaccination history was recorded by the panel physician, I don't believe they need to return to a civil surgeon.
