I m preparing to send form/papers to AOS (adjustment of status) i have filled out the following forms :

g-325a

g-1145

i-134 and i-864

i-485

i-765

I-94

copie of visa copie of passport

I want know is there more forms or papers needed also i filled all of that expect on the i-765 i do not know what to put in eligibility category question should i put in it (a 16)

I wish someone can help us .

No prob but make sure u add 2015 as well as I am sure you got ur tax return by now I am guessing, when I filed my work my husband didn't get his tax return for 2015 at the time we filed

If your fiancé/spouse's birth certificate is not in English it will need to be translated. If you are fluent you can do just make sure to translate anything that's not in English. That's what I did, even the stamps lol. If not, someone went to rev.com and had it done within the day between $25 and $35

no i mean the package do i send it to uscis that my i129 f got prossed at or send it to somewhere else and do i need to send an I134 too with the package i m sorry for all this questions

You don't send an I-134 with your AOS package, you send the I-864. As to where you mail the package, you'll find the address here https://www.uscis.gov/i-485-addresses. Btw, your timeline is a bit confusing. It indicates that you received your K-1 visa and entered the U.S. in January 2016, but that your marriage was in November 2015.

10/02/2015: CEAC status changed to "In Transit"

10/09/2015: Consulate acknowledged receipt via phone; Hardcopy from US DoS with case number.
10/19/2015: Received information from Consulate.
10/28/2015: Medical Exam
11/17/2015: Interview...rescheduled due to computer problems. New date as yet unknown.
11/18/2015: Called in for interview and APPROVED!

11/24/2015: Picked up visa/passport and sealed packet from the Consulate! <happy dance>

02/14/2016: Arrived in the U.S. (POE was Bermuda)...CO was really nice!
