On the N-400 form.... part 2....number 2....they want her name EXACTLY as it appears on her PermRes card.

As follows....

Family name: Given name: Middle name:

However, her PR card has her First name..... and FIRST LETTER of her MIDDLE name under "Given name" on the PR card.

As follows on PR card....

Surname:

Doe

Given name:

Jane M

Is it just because they do not have a spot for middle names on the PR cards?

On the N-400 form should i put her middle name on the "Middle name" part? or do they want Jane M in the given name part and leave the middle name part blank??????

Could someone look at their application and see what they put? Please!

THANK YOU!

Whenever it didn't say "Name exactly as Permanent Resident Card", I wrote my full middle name.

Did you put your Middle initial with your First name (Given name) where it said, "Name exactly as Permanent Resident Card?"

Middle name or middle initial go in the Middle Name field.

Look at the back of her green card in the coded section at the bottom. Does it have her full middle name there or just the initial? If it's just the initial, then her middle name is only an initial in their records and that's what you should write on the application. If it's her full middle name, then that's what's on their records (they only put the initial on the front of the card); in this case I would write her full middle name, but others only list the initial without any problems.

In reality, it doesn't matter because they'll have a copy of the green card and if you list only the initial, make sure to list the full middle name under Other Names Used so that all names will be checked in the background check anyway.
